Victims of Human Trafficking

Safe-Screen is committed to complying with the Consumer Financial Protection Bureau’s (CFPB) amendment to Regulation V of the Fair Credit Reporting Act (FCRA) that assists consumers who are victims of human trafficking. 

Instructions for submitting a request to block adverse information that resulted from human trafficking:

The Fair Credit Reporting Act (FCRA) requires the following information from any person requesting a block of information on their background check report.

  1. Proof of Identity
    1. Provide your full name (first, middle, last, suffix)
    2. Any previously used names
    3. Current and/or recent full address (street number and name, apartment number, city, state, and zip code).
    4. Social Security Number
    5. Date of Birth
    6. Proof of identity. Copies of government-issued identification documents, utility bills, and/or other methods of your identity.
  2. Trafficking Documentation, include at least one of the following:
    1. A determination of victim status from a federal, state, or tribal government entity;
    2. A determination of victim status from a non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination; or
    3. A self-attestation that you are a victim of trafficking. The statement or an accompanying document must also be signed or certified by a government entity, a non-governmental organization or human trafficking task force, or a court.
  3. Identification of the adverse information that resulted from human trafficking that you wish to block.
  4. Contact Information. Full mailing address and/or email address for correspondence, and your preference.
